A vendor application can be obtained by contacting the Procurement Division at (229) 431-3211 or by printing a copy from the website. When completed, the application, W-9, and E-Verify documents may be mailed, emailed, or faxed to the Procurement office. A List of Commodities is available on the website to complete the application. Please adhere to the City's insurance requirements, as outlined in the link provided below. If you are a current vendor and would like to submit an address change, please complete a Vendor Update Form and a new W-9 Form.
